Free physicals will be give tomorrow, Wednesday, May 16th, for students in grades 6th-11th who are interested in participating in sports for the 2018-19 school year. On May 16th, the school will transport the students to the Westville NEO Health Clinic as long as the student has a signed OSSAA Physical Form and Consent form.
On the consent form, please write the grade that the student will be in during the 18-19 school year. The link for the physical and consent is below.
Physical Form and Consent to Print and Return
NEO Health is offering a 2-week period for students to have their physicals done at no cost to the parents. After May 30th, the cost will be $25 and by appointment only.
If your child plans to participate in any of these sports, they are required to have a physical on file:
- Cheer - Competitive and sideline
- Football
- Volleyball
- Softball
- Basketball
- Baseball
- Soccer
- Track
