Vizavance, formally known as Prevent Blindness Oklahoma, will be doing vision screening for PK-12th grade this year. Vision screenings will be on Monday, October 16th for PK-6th grades and Tuesday, October 17th for JH and HS. If you DO NOT want your child to be screened, please print the opt-out form from the link below, sign and send it to school as soon as possible. I also have forms in my office.
